Introduction At Curate Care, your privacy, dignity, and trust are central to the way we deliver our services. As an NDIS provider, we are committed to upholding your legal rights to privacy and ensuring your informed consent is always obtained and respected. This Privacy and Consent Policy outlines how we collect, store, use, and share your personal and sensitive information, and your rights regarding this information.

1. Collection of Personal Information We collect personal and sensitive information that is necessary for us to deliver our services. This may include:

Your name, contact details, date of birth, and preferred communication methods

Your NDIS plan details, funding information, and participant number

Health and disability information relevant to the supports we provide

Information about your goals, preferences, and informal supports

Service history and interactions with Curate Care

Details about third parties involved in your care (e.g., support coordinators, advocates)

Directly from you or your authorised representative

From NDIA or other service providers with your consent

During meetings, referrals, or service delivery activities.

2. Use and Disclosure of Personal Information Your information is used to:

Coordinate and deliver services relevant to your NDIS plan

Submit claims, manage budgets, and ensure quality support

Fulfil legal obligations under the NDIS framework and other relevant laws

3. Consent for Sharing Information We will seek your informed consent before sharing your information, except in exceptional circumstances as outlined above. You have the right to:

Nominate individuals or organisations to act on your behalf with the NDIA

4. Verbal Consent Where written consent is not practicable, verbal consent may be accepted. Our staff will ensure you understand the proposed disclosures before proceeding.

5. Video/Audio Recording You will be informed and consent obtained before any video or voice recordings are made as part of service delivery.

6. Privacy Safeguards We take all reasonable steps to protect your personal information from misuse, interference, loss, and unauthorised access. Information is securely stored electronically and in physical formats, and access is restricted to authorised staff.

7. Access and Correction You may request access to the personal information we hold about you. If any information is incorrect, you may request it be amended. We will respond to requests within a reasonable timeframe.
